Special Dietary Needs

We want to make sure your child can safely eat meals at school.

If you believe your child requires a special diet based on a disability, food allergy or other medical condition, you must fill out a "Statement to Request Accommodations for Special Dietary Needs" form (links below.)

This form must be used when requesting any dietary accommodation. The form contains 2 parts. Part 1: To be completed by Parent/Guardian. Part 2: Dietary Restrictions section. To be completed by a Medical Doctor. 

This form only must be submitted ONE time during your student's school enrollment.  This form will remain on file and meal accommodations will remain in effect until the student leaves the Mitchell County School System or we receive written notice of a change. If for some reason there is a change in your child’s medical needs, we will need a statement from a medical professional formally changing or discontinuing the previously ordered dietary accommodations. Once the form has been completed, please return it to the school nurse at your child's school, who will then notify the school nutrition director and cafeteria manager of the dietary request.
